First, identify the problem the plan would address

The upper-jaw width finding is the starting point. The orthodontist explains the discrepancy being considered and how it relates to the teeth and bite. A wish for straighter front teeth, by itself, does not identify the underlying width problem.

Treatment goals make that finding useful. The explanation should connect the proposed expansion with the intended contribution, then identify any tooth-position or other bite goals that need a separate part of care. This keeps an appliance preference from taking the place of a clinical recommendation.

The tissues around the teeth matter too

Assessment includes the relevant jaw anatomy and the condition of the gums and bone supporting the teeth. MARPE research describes tooth tipping and supporting-bone or gum changes, so these tissues matter when the clinician considers both the starting situation and the effects to monitor.

That is why two people asking about the same appliance can need different explanations. A method's name does not describe either person's tissue findings, goals or tradeoffs. The clinician brings those pieces together before recommending an approach.

Understand what any additional record is meant to clarify

Examination, periodontal assessment and appropriately indicated imaging provide different information. A useful records discussion names the unresolved question and explains how the proposed information can help with the decision or progress assessment.

The 2026 review describes complementary roles for clinical findings and imaging. It does not establish that every visitor needs the same scan or scan schedule. At SMILE-FX®, digital planning supports the doctor's explanation, and imaging is based on clinical indication.

Compare options by their intended contribution

The alternatives discussion explains which approaches are reasonable for the actual finding, what each aims to accomplish and the tradeoffs. A tooth-movement plan and an upper-jaw-width plan have different targets, even when they are connected in one course of care.

Surgically assisted expansion can be a separate width-correction option for clinical consideration. Whether it or another approach belongs in a particular plan requires assessment. Surgical assistance for expansion should also be distinguished from orthognathic jaw correction for a different jaw relationship; the continuing-plan chapter explains where the coordinated jaw-care discussion belongs.

A worthwhile explanation gives the reason for recommending an approach and the limits it leaves. It also explains the alternative pathway to discuss if expansion does not achieve its intended contribution. It does not promise that choosing MARPE removes every possible need for another procedure.

Fictional example: Dani understands why the assessment is unfinished

This is an invented example. Dani wants the front teeth aligned. The orthodontist identifies an upper-jaw-width discrepancy and explains that the gums and supporting bone need assessment before choosing among expansion options. MARPE has been introduced as an option to evaluate, without a device decision yet.

The width finding gives expansion a possible purpose in Dani's care. The front-tooth goal identifies a separate tooth-movement discussion. The supporting-tissue assessment contributes information about the starting condition, relevant tradeoffs and what would need monitoring; it is part of making the recommendation specific to Dani.

Dani can now explain why the assessment has not ended with an appliance choice. The clinician still needs to combine the relevant findings and goals, compare reasonable approaches and explain the recommendation. The example establishes that decision's purpose without choosing a device, an age rule or an imaging prescription for Dani or the reader.

A study's participants are not an eligibility test

The 2021 review included selected late-adolescent and adult groups and found very-low-certainty evidence. The 2026 tissue review found low-to-very-low-certainty evidence across varied methods. Their participant ages and reported results do not supply a universal age cutoff, a self-test or an individual success prediction.

For an in-person discussion, the useful starting point is your actual concern and any existing proposal or records you already have. The SMILE-FX® team can confirm the appointment and consultation scope. Current patients should use their treating team's established route for questions about an existing plan.
